For members currently contributing to a retirement system, address changes must be done through your employing agency. Contact your HR/payroll office to update your address.

| Designation of Beneficiary-NonStatutory | The NonStatutory membership form is for new hires and for current PERS, JRS, GWPORS, and SRS members who wish to change their designated beneficiaries. |
| Designation of Beneficiary-Statutory | The Statutory membership form is for new hires and for current HPORS, MPORS, FURS, and VFCA members who wish to update their statutory beneficiaries or change their designated beneficiaries. |
| Address Change Form for Inactive Members | If you are no longer contributing but still have an account with MPERA, use this form to notify us if your address changes. If you are contributing to your account, do not use this form. Instead, notify your payroll clerk or HR person of your address change. They will notify us. |
| Authorization for Release of Confidential Information | Authorization to waive the right of confidentiality in order to allow a named individual or entity to have access to retirement information. |
| Election and Certification to Purchase Absence | This form is for the certification and purchasing of service credit for absent time covered under the Uniformed Services Employment and Re-employment Act (USERRA), and Workers' Compensation. |
| Estimate Request | If you are considering retirement or participating in DROP, fill out this form to request an estimate. For Defined Benefit participants only. |
| Military Time Buyback Letter | If you wish to buy your military time, complete this letter and return it to MPERA. |
| Name Change for Inactive or Retired Members | If you are an inactive or retired member and have changed your name, please complete this form and return it to MPERA. If you are a current member, please submit name changes through your employer. |
| Retirement Fund Information | If you are leaving employment, you have several choices regarding your retirement funds. Please read the information, then call 877-275-7372 to talk to us regarding your refund options. |
